Great opportunity for a Social Worker or Nurse who wants to help Seniors and their families age well. This position is both in the field as well as virtual as you will be helping people throughout our company. Successful candidate will have five plus years working with Seniors in the community, have great assessment and communication skills and be able to be proactive.

We are currently looking for a Care Manager to join our team in New York, Supporting the Caring People Queens office!

Roles and Responsibilities:
- Care Coordination
- Medical oversight
- Interfacing with medical personnel
- Advocacy, information and referrals
- Work with interdisciplinary team members to assist our geriatric clients
- interventions and necessary assistance
- Advocate for client in all environments
- Coordinate community resource referrals appropriate for each client
- Timely maintain and input accurate case records into our software database
- Be part of a cohesive care management team and interact with Operations Team.
Qualifications/Requirements:
- Minimum of 2+ years experience with the geriatric population in the community
- Master’s Degree highly preferred, but not a requirement (Social Worker)
- Recognizes industry trends and problem solves
- Self-motivated and work with own initiative, professional and positive approach, commitment to customer service
- Respectful of company and client confidentiality
- Personalized and compassionate service with Person-Centered approach
- Ability to provide non-directive guidance and facilitate constructive relationships
- Demonstrates Flexibility and Accessibility – This includes outside business hours if needed
- Quality control – care management services adhere to Aging Life Care Associates Standards of Practice and Pledge of Ethics
- Strong in building relationships, team player and able to communicate at all levels
- Ability to provide coordinated communication between family members, doctors and other professionals, and service providers
- Must have great communication and listening skills, be a team player and possess the ability to interact with a variety of individuals.
- Ideally must have relationships with various resources in their community.
