Overview:
We are seeking a highly skilled and compassionate Registered Nurse (RN) to join our team as a Clinical Liaison specializing in phone triage. As a pivotal member of our healthcare team, you will be responsible for providing expert clinical assessment and guidance to patients and their families over the phone. Your role will be crucial in ensuring efficient communication, accurate assessment, and appropriate coordination of care, ultimately enhancing patient outcomes.
Responsibilities:
- Phone Triage:
- Perform comprehensive assessments of patient symptoms and concerns over the phone.
- Utilize critical thinking and clinical expertise to determine the urgency of each situation and provide appropriate recommendations.
- Offer clear and concise instructions to patients and their families regarding self-care, medication management, and when to seek further medical attention.
- Document all interactions accurately and thoroughly in electronic medical records (EMR) systems.
- Clinical Liaison:
- Serve as a liaison between patients, their families, and other healthcare professionals, facilitating effective communication and collaboration.
- Coordinate with physicians, nurses, and other members of the healthcare team to ensure seamless continuity of care.
- Provide education and support to patients and their families regarding their healthcare needs and treatment plans.
- Patient Advocacy:
- Advocate for patients’ needs and preferences, ensuring that their voices are heard and their concerns addressed.
- Identify potential barriers to care and work collaboratively with the healthcare team to overcome them.
- Quality Assurance:
- Adhere to established protocols, guidelines, and standards of practice in all interactions and assessments.
- Participate in ongoing quality improvement initiatives to enhance the efficiency and effectiveness of phone triage services.
- Maintain compliance with regulatory requirements and organizational policies.
Qualifications:
- Valid Registered Nurse (RN) license in the state of practice.
- Bachelor of Science in Nursing (BSN) preferred.
- Minimum of 2 years of clinical nursing experience, preferably in an acute care or ambulatory setting.
- Strong assessment and critical thinking skills, with the ability to make rapid and accurate clinical judgments.
- Excellent communication and interpersonal skills, with the ability to convey complex medical information in a clear and compassionate manner.
- Proficiency in using electronic medical records (EMR) systems.
- Ability to work effectively both independently and as part of a multidisciplinary team.
