Intake Data Specialist for R3 Program
Salary Range: $45,000-$55,000 yearly.
Schedule: 830am-5pm Mon-Fri.

JOB SUMMARY:
We are currently seeking an Intake/Data Coordinator to assist our Behavior Health/R3 Program. In this position, you will conduct initial intake assessments, ensure potential clients are eligible for care, and enter their data into our system. You will also assign case management, counseling, and care coordination of the clients and families ensuring all referrals for the R3 grant and PLCCA wrap-around services both external and internal. In addition, you will update the existing client charts as well as create new ones. Applicants should have some experience in an administrative role, preferably in a Mental Health setting, and possess excellent data entry skills.
INTAKE COORDINATOR DUTIES AND RESPONSIBILITIES:

Conduct initial intakes to ensure eligibility of potential clients.

Coordinate of all program records and support for the evidence-based program data and will facilitate all pre and post-surveys.

Facilitate all pre/post surveys.

Obtain medical histories and personal information of new clients.

Responsible for facilitating all internal and external referrals for the R3 Grant.

Update all records of existing clients.

Review documents for accuracy and completeness.

Prepare new client paperwork and complete assessment.

Assist with billing as needed.

Training of sub-grantee and staff on data reports.

Develop a tracking system.

Additional duties as needed.
INTAKE COORDINATOR REQUIREMENTS AND QUALIFICATIONS

High school diploma or GED certificate

Associate or bachelor's degree (preferred)

Client scheduling experience

Experience with health insurance a plus (Medicaid/MCO)

Computer and data entry skills

Bilingual communication skills a plus.
