Position Summary: Community Homes is a non-profit that provides community-based housing in adult family homes and shared living environments in East King County. We have 49 residents with developmental disabilities in 11 neighborhood homes with their live-in caregivers. We provide ongoing wrap-around support for our residents, caregivers, and families and develop partnerships to provide opportunities for our residents to participate in their communities and live a well-rounded adult life.
Utilizing our Community Homes framework creates a higher housing stability rate over the long term. Many residents have lived in one of our homes for over 15 years. Being active in our internal community, working or volunteering, and participating in activities or events in the greater community increases our long-term success of living more independently and finding self-fulfillment.
Our new care manager will increase resident engagement through in-house activities and participation in their community, provide support during transition periods, and cultivate and establish ongoing partnerships with community organizations to enhance resident involvement. Our new manager will learn from our residents what is most important to each of them, collaborate with our caregivers and families, and execute a plan. There will be many opportunities to establish collaborative partnerships and find interesting ways to create community and enrich our residents' lives.
Responsibilities:
Resident Community Engagement:
- Plan and execute in-person activities and events for residents, both individually and in groups, within the home and in the broader community
- Cultivate and nurture ongoing partnerships with community organizations that support our efforts to expand community, work, and volunteer access
- Facilitate engagement of residents with local organizations
- Coordinate resources, advocate for client's needs, and make referrals as needed
- Support metro transportation and ACCESS use
- Train and provide ongoing support for volunteers working with our residents
- Foster working relationships with our caregivers and families to support resident engagement
- Coordinate additional staff support for resident activities
- Provide training to support staff to enhance best practices in activities for residents
Grant Management:
- Manage all grant requirements, including data collection, reporting, and feedback required for grant funding
- Work with the Director of Finance to compile necessary documents to meet quarterly and yearly requirements
- Collect pictures and videos to document work completed
CH Staff Responsibilities:
- Plan and support celebrations, events, and enhance resident engagement
- Active participant in community and fundraising events
- Attend staff meetings, strategic and tactical sessions, and other meetings when requested
- Other tasks as assigned
Minimum Qualifications:
- 2+ years experience planning and facilitating activities for groups and individuals living with disabilities
- 1+ years of partnership development with various community entities
- 1+ years of experience successfully accessing and navigating disability services (ex. DDA, SSI, SSDI, Section 8)
- Lived experience with individuals with disabilities and passion for supporting individuals in the community
- Valid driver's license and reliable transportation
- Sound judgment with safety and a high level of discretion
- Excellent verbal and written communication skills
- Excellent reporting skills specific to the requirements of job responsibilities
- Effective interpersonal skills to develop collaborative internal and external working relationships
- High levels of organization and prioritization
- Ability to be self-directed and accountable
Physical Requirements
- Moving throughout different landscapes at the resident homes and in the community
- Use of the computer for planning and reporting
- Standing and stooping and physically providing support to residents using a walker, walking on uneven surfaces and navigating unfamiliar environments
- Ability to lift to 40lbs
