Safe Vision Family Program is seeking a dedicated Creole-speaking Health Care Support professional to join our team in Manhattan and Bronx, NY. The ideal candidate will have experience providing health care support services to youth and families from diverse backgrounds, particularly those who speak Creole.
Responsibilities:
- Assist clients with accessing and navigating health care services
- Provide information and education on health-related topics
- Advocate for clients and help them communicate with medical providers
- Coordinate appointments and follow-up care as needed
- Handle inbound customer inquiries via phone, email, or chat
- Perform data entry tasks accurately and efficiently
Qualifications:
- Bachelor's degree in health care administration, public health, social work, or related field
- Fluency in Creole and English
- Experience working in a health care setting or providing health care support services
- Knowledge of community health resources and services in Manhattan and Bronx, NY
