Position Summary:
This is a full-time, Monday - Friday 8:30AM-4PM, salaried, exempt direct care position. The Licensed Practical Nurse (LPN) will be responsible for care tasks to ensure that the Individual receives the comprehensive and personalized care he/she needs. The LPN will work in multiple residential sites and have the following responsibilities:
- Grooming and bathing Individuals with low mobility. Helping with toileting or changing briefs and diapers.
- Following up to ensure all needed medical equipment is operating properly in the residential site and all needed supplies are available.
- Assuring dietary needs are met in accordance with physician’s orders.
- Responsible for helping in the creation of balanced, nutritious meals that reflect Individuals’ tastes and preferences as well as the creation of meals suited to specific Individual health needs and requirements.
- Monitoring vitals of blood pressure, temperature, dietary needs, skin integrity, medication regimes and side effects, Individual’s behavior and reporting results to the Program Director/Program Specialists and agency LPN for Health Risk Screening Tool Assessment.
- Working as a team with agency residential staff, agency Licensed Practical Nurse, Program Specialist/Supervisor and Program Director to obtain quality medical care. Ensure all doctor’s recommendations are followed.
- Identifying health risks and problems to facilitate appropriate medical care follow-up.
- Assisting Individuals with mobility needs, transferring from wheelchair or walker, transferring in/out of a wheelchair into/out of bed, transferring in/out the shower.
- Turning or repositioning Individuals in bed to prevent skin breakdowns and bedsores.
- Changing dressings and bandages and monitoring the healing process.
- Accompanying Individuals to specialized medical appointments such as cardiologists, neurologists, endocrinologists.
- Striving to enhance and maintain Individual privacy in the home.
- Ensuring a clean and sanitized living environment.
- Improving the Individual’s quality of life.
- Completing all required documentation.
- Other related duties.
Compliance
- Attending a minimum of 24 hours of in-service training per year.
- Completing all mandated Office of Developmental Programs core curriculum training.
- Participating in trainings, educational seminars and workshops addressing residential living and prevention of health risk factors and/or the delivery of human services.
- Serving as an Advocate for Individuals to assure rights and choices are safeguarded, especially medical rights.
- Following the Incident Management/Risk Management protocols in recognizing and reporting incidents, cooperating, and participating in incident investigations
Essential Functions
- Must be able to push/pull/lift up to 50 pounds.
- Must be able to sit/stand/walk for extended periods of time.
Requirements
- Valid Driver's License
- Valid PA LPN license
- At least one year of experience in the medical, mental health, behavioral or Intellectual Disabilities field
