Description: DAMIAN FAMILY CARE CENTERS, INC. is a Federally Qualified Health Center with 14 community based health centers located in the metro NYC area, Suffolk County and Upstate, NY, providing primary care, behavioral health and specialized care at all sites .
Position Summary:
- Assumes responsibility and accountability for planning, organizing, coordinating, directing, monitoring and evaluating the provision of care and services for the center(s) in the designated area. Will coordinate clinical needs of the clinic in collaboration with the Director of Operations and patient providers.
- Collaborates with the Director of Operations and the Director of Nursing in promoting quality patient care to the communities we serve through the appropriate utilization of resources and coordination of professional and ancillary staff.
- Assumes responsibility for ensuring the daily needs of the center in order to ensure compliance with the policies and protocols of DFCC are met as well as the regulations of the Department of Health, the Federal Government, and OSHA.
- Acts as a liaison with outside agencies to ensure that the rendered care being rendered at the center(s) is appropriate.
Essential Functions:
Facilitates, plans, organizes and directs an interdisciplinary team in promoting the philosophy and mission of Damian Family Care Centers, Inc. to ensure the provision of safe, quality and cost-effective care to the communities we serve.
Utilizes the CQI process to identify opportunities for change, analyze data, systems and processes and develop plans for improvement in collaboration with the Director of Operations and the Director of Nursing. Actively participates in all activities that deals with Quality of Care issues.
Demonstrates initiative in facilitating the development and implementation of systems, processes and standards of care that are consistent with current, State and Federal statutes, grant requirements, and health care trends.
Assists the Director of Operations in planning and defining the health center’s short and long-term goals and developing strategies that will improve the provision of services and operational efficiency.
Demonstrates the ability to apply acceptable leadership skills in setting priorities, delegating appropriately, managing time, resolving conflict and initiating change.
Actively participates in various DFCC internal committees as well as external committee meetings in conjunction with DFCC partners.
Verbalizes a basic understanding of budgeting, staffing and reimbursement methodologies and utilizes that information to provide health center services.
Regularly tours and meets with staff at the DFCC sites in the designated region to promote collaboration and communication; assesses staff performance; monitors organizational efficiency, assesses compliance to established standards of care and DFCC policies/procedures.
Maintains a cooperative relationship among established partners by communicating information; responding to requests; building rapport; participating in team problem-solving methods.
Demonstrates a commitment to professional growth by attending educational seminars and keeping current with health care trends.
Demonstrates flexibility, creativity and initiative in performing all aspects of the management role and effectively utilizes management concepts to develop and motivate staff to achieve maximum potential and productivity in the delivery of care.
Ensures that time schedules are prepared in accordance with approved staffing methodologies and facilitates the appropriate allocation of staff in the designated region.
Protects patients and employees by developing and interpreting infection control policies and procedures; enforcing accurate medication administration and storage procedures.
Assumes responsibility and demonstrates initiative in identifying the learning needs of staff and planning organized educational programs that meet the competency and mandatory educational requirements for all staff.
Facilitates the selection of new employees in accordance with hiring policies, job requirements, qualifications, licensure and approved positions.
Maintains a well-organized and comprehensive orientation program for new employees. Responsible for staff training of health center workflows and procedures and ensuring compliance to DFCC annual mandatory educational programs and recredentialing process.
Evaluates staff performance on a timely basis and in accordance with agency policy. Initiates the employee disciplinary process, when required and, as directed, in consultation with the Director of Operations.
Demonstrates a commitment to building a supportive work environment that fosters collaboration and communication between all health care team members, projects a positive image to the communities we serve and demonstrates respect for the cultural and religious diversities and needs of patients, families and care providers.
Acts as a liaison between the center and the referring agencies, such as our residential communities’ affiliations, referring or patients to us to ensure that the center is meeting the needs of the patients and their affiliating programs.
Assumes role of patient advocate and conforms to policies and regulations governing patient rights, confidentiality and the HIPAA Privacy & Security Rules.
Ensures that a safe, clean and therapeutic environment is maintained at the center(s) in accordance with regulatory requirements and the DFCC environmental and physical plant program.
Demonstrates competency in the utilization of Health Information Technology to perform essential job functions, which include but are not limited to (1) documentation in the EHR, (2) Transmission of EHI, (3) Retrieval of EHI
Maintains a consistent and flexible pattern of attendance in accordance with assigned schedule and reports to work on time.
Conforms to agency guidelines regarding dress code; is well groomed and neat. Maintains good personal hygiene.
Performs all related duties, as assigned or unrelated duties, as requested, during emergencies.

Qualifications:

Bachelor’s Degree in healthcare administration or related field required. Master’s Degree in Business Management or related field preferred.

A minimum of five years’ recent medical practice experience two years in a supervisory capacity required.

Knowledge of healthcare administration including compliance regulations.

Ability to work independently and part of a team; prioritize tasks. Must have the ability to interface with participants/patients as well as all levels of staff.

Ability to interact effectively and professionally with persons from diverse cultural, socioeconomic, education, racial and ethnic backgrounds.
