We are looking for a passionate Patient Care Manager (RN) to join our leading team of hospice care professionals and make a difference, by listening, caring, and serving.
POSITION SUMMARY: Coordinates and supervises an interdisciplinary team of staff to assure the continuity of high-quality care to hospice patients assigned to the team's area in accordance with physician prescribed plan of care, and all applicable state and federal laws and regulations.
QUALIFICATIONS:
- Requires college degree; strongly prefer a Registered Nurse/BSN
- 2+ years of hospice experience and supervisory experience required.
- Experience in a Charge Nurse role will be considered to meet supervisory requirement.
- Knowledgeable of hospice regulations
- Proficient skill using a computer is required; Microsoft office suite experience required.
- Good communication skills and ability to travel independently
