Purpose of this position
The purpose of this position is to provide direct care to residents, under the supervision of a licensed nurse, in accordance with facility policies and procedures and report resident needs and concerns to a licensed nurse.
To provide each residents with routine daily personal care and assistance in accordance with the resident's individual assessment and care plan, as directed by your Charge Nurse or Supervisor.
The assisted living care manager is responsible for a consistent group of residents based on the designated care manager concept. Each assisted living care manager is responsible for providing the hands-on care, both physical and emotional, as outlined in each resident’s Individual Service Plan. The assisted living care manager is also responsible for ensuring that daily housekeeping and laundry tasks are completed. The assisted living care manager supports the residents by serving in the dining room and by facilitating or participating in activities and outings. The assisted living care manager is responsible for effectively communicating with residents and families, and for notifying the Resident Care Coordinator or Wellness Nurse of any changes in condition or concerns. The assisted living care manager is responsible for both reading the Daily Log and for making appropriate entries as needed. The assisted living care manager is responsible for performing all job requirements in a manner which exemplifies the Merry Heart’s principles of service.

- Ability to read, write, and speak English
- Ability to prioritize, is flexible, and organized
- Must possess a valid C N A or CHHA certification in the state of New Jersey
- Must be enrolled in or has completed a state approved Nurse Aide/Home Health Aide training course.
- Must be CPR certified.

Pay: $17.13 - $19.00 per hour
