Job Summary
We are seeking a dedicated and compassionate Nurse to join our Home Care team. As a Nurse, your expertise and attention to detail will be crucial in ensuring the well-being and health maintenance of our clients.
Duties:
- Assess the appropriateness of home care for prospective clients.
- Regularly visit clients in their home every 3 months; monitor vital signs, develop plan of care
- Regimen, and appraise medical supply needs and facilitate procurement of same.
- Supervise the home attendant, making recommendations and teaching the home attendant and client techniques for better personal home care.
- Make emergency visits when the special skills of a Nurse are needed.
- Evaluate clients for implementation of the personal emergency response system.
- Visit clients in their home for supervisory, referral and/or incident review on an as needed basis.
- Case management - You will coordinate with multidisciplinary team to ensure continuity of care, wellness promotion and home safety of hospitalized, post hospitalized, and clients with safety concerns.
- Responsible for evaluating all home attendants once a year in the field.
- Screen clients for COVID and Flu and document accordingly.
- Perform any other tasks deemed necessary and proper for the enrichment of the program by the Director of Elder Care and the Supervisor of Patient Services.
Qualifications/License/Certification
- Valid RN license in New York (Required)
- BLS Certification (Preferred)
- Proven experience as a Nurse in a hospital or home care setting 3 years or more
- In-depth knowledge of physiology, medical terminology, and disease processes
- Strong communication and interpersonal skills
- Excellent problem-solving and critical thinking abilities
- Computer Savvy
Rates (PER VISIT ONLY):
- Supervisory and In-home Evaluations $60
- Initials and Reassessments $75
