YOUTH CONTINUUM, INC is a nonprofit agency in New Haven, CT . We are inclusive, rewarding, engaging and our goal is to make sure our area’s most vulnerable young people have a safe bed, critical support services, and a path to independence. We serve 500 youth annually who are experiencing homelessness, housing insecure, or exiting the state’s Child Welfare system and transitioning to adulthood and independent living. We are seeking qualified and passionate Case Manager's to work in our:
- Rapid Rehousing
- Permanent Supportive Housing
- Transitional Living Program
- and Access Programs
Our Case Manager Positions work with homeless and at risk youth. We have both Full-time, and part-time positions available. Monday - Friday 9:00 a.m. to 5:00 p.m. MUST BE FLEXIBLE.
We are looking for someone to be a key member of the program team participating in the care, behavioral intervention and support of at-risk youth. Monitor, develop and coordinate all aspects of the clients’ program participation, including but not limited to Housing Search & Stabilization, Educational, Vocational, Social, Financial, Recreational and Interpersonal Development. Implement program goals and behavioral interventions as set by the Program Director/Manager. Provide documentation to support program participation. This position works in collaboration with Program Personnel and Community Resources to provide a successful transition to self-sufficiency and prevent long-term dependence on the social service system.

QUALIFICATIONS:
EDUCATION and/or EXPERIENCE
- Bachelor’s degree is required in a related field and must have two to three years experience working with at risk youth, homeless youth, foster care youth, and/or young parents in a transitional and/or independent living environment or
- Relevant work experience of 3+ years will be considered in lieu of a Bachelor’s degree.
LICENSES AND CLEARANCES:
- A current CT Driver’s License, Registration, and Insurance.
- Maintain a clear driving record.
- Pass state and federal criminal check.
- Pass a DCF background check
- Maintain a clear DCF abuse and neglect record.
RESPONSIBILITIES:
Essential Duties and Responsibilities include the following. Other duties may be assigned.
- Provide referral, advocacy, crisis intervention and counseling services to youth.
- Provide housing search and support, landlord engagement, etc. to secure housing.
- Liaison between Youth Continuum, Coordinated Access Network, landlord, etc.
- Provide Case Management services, including liaison with community resources.
- Complete all required documentation including case notes and HMIS, ensuring data quality and completeness.
- Work closely with the property manager and/or landlord to ensure well maintained properties & HQS compliance.
- Provide supervision to ensure the safety and stability of program participants.
- Monitor program participants via announced and unannounced visits.
- Provide on-call coverage to program youth, 24 hrs/7 days.
- Participate in facilitating monthly groups with a focus on life skills development.
- Develop a written Service Plan with program participants outlining individualized goals including, but not limited to, employment, education, housing, budgeting and discharge planning.
- Monitor and document progress on goals.
- Assist clients to obtain appropriate, safe and permanent housing prior to discharge.
- Establish/maintain a network of landlords within the service area.
- Assist with medical needs, as needed and as applicable.
- Assist in developing and monitoring client budget.
- Maintain/oversee all client related documents and files; maintain confidentiality.
- Complete intake procedures with clients.
- Participate in ongoing agency training and staff meetings.
- Provide coverage at other programs in the event of an agency emergency.
- Maintain appropriate boundaries with program participants.
- Complete funding source reports and actively participates in grant outcome monitoring.
- When appropriate, provide after-care services.
- Perform other duties as assigned, i.e. clean vacated apartments, HQS inspections, agency committee assignments, etc.
