PRN per Visit rates from $85-$115 depending on function performed. New Home Health agency needs coverage in Ft.Worth, Dallas County, Collin County, Grayson County and Rockwall County
POSITION SUMMARY
Primary functions are to provide nursing care for Home Health agency clients of all ages in their place of residence, coordinate care with the interdisciplinary group, patient/family and referring agency; and assumes the responsibility for coordination of care. Works closely with DON to ensure timely response to calls for assistance and prompt charting for billing. Oversees resident care including medication administration, obtaining resident vital sign and weight, and reporting any changes in condition to physicians, health care proxies and supervisors.
ESSENTIAL DUTIES AND RESPONSIBILITIES

- Working as a PRN (per Visit) contractor, Completes initial and ongoing assessments to identify the physical, psychosocial, and environmental needs of home health patients. Completes assessments at appropriate time points, including Outcome and Assessment Information Set (OASIS), or other assessments as appropriate to the patient.
- Develops, initiates, and revises the plan of care in partnership with the patient, representative (if any), and caregiver(s) as necessary to ensure quality and continuity of care. Initiates appropriate preventive and rehabilitative nursing procedures. Refers to other services as needed. Provides patient and caregiver education.
- Prepares clinical and progress notes. Completes appropriate documentation in a timely manner.
- Regularly reevaluates the patient's nursing needs and evaluates the outcomes of care.

Educational Requirements and Experience:
- Active Registered Nurse license to practice nursing in the state of Texas.
- Two years experience as a Registered Nurse in a clinical care setting preferred, hospice or home health preferred.
- Graduate from an accredited School of Nursing.
- Current CPR certification.
