UIC Division of Specialized Care for Children is a statewide social services organization that provides care coordination to children in Illinois with special healthcare needs. This full-time position starts out with 25 vacation days, 12 sick days, and over a dozen paid holidays. Currently, this position will work from home on most days and go into the office or visit clients as needed. Illinois residency is required.

Position Summary:
The DSCC Medical Social Consultant provides care coordination services to families eligible for DSCC Home Care services within a particular geographic area. Provides consultation to other members of the multi-disciplinary team utilizing skills and knowledge acquired from academic training and professional experience as a Social Worker.
Duties & Responsibilities:
- Provides care coordination services: Develops a care coordination plan that is participant / family-centered, community based and coordinated. Facilitates the implementation of the care coordination plan. Monitors the care coordination plan activities. Revises the care coordination plan to address the changing and ongoing concerns and priorities of the family. Coordinates all available health care benefits.
- Identifies children eligible for DSCC services: Assists in determining general eligibility for each assigned case along with Program Coordinator Assistant. Determines medical eligibility for each assigned case. Assists in the completion of the financial application process for each assigned case along with Program Coordinator Assistant.
- Participates as a member of the Regional Office multi disciplinary team: Complies with University, Division and Regional Office policy and procedures. Provides discipline based expertise to the Regional Office multi disciplinary team. Maintains psychosocial expertise in support of the Regional Office multi disciplinary team. Complies with state and federal regulations related to children enrolled in the Home Care Program.
- Promotes interagency collaboration and an organized network of integrated services: Provides community education programs regarding DSCC services. Participates in developing and/or implementing a networking plan for the Regional Office.
- Participates in participant / family-centered, community based clinics (if applicable): Coordinates all assigned DSCC clinics. Participates as a Medical Social Consultant team member.
- Participates in special projects impacting DSCC: Participates in organizational committees/projects on a regional or statewide basis. Participates in DSCC sponsored interagency programs.
- Perform other related duties and participate in special projects as assigned.
Minimum Qualifications:
Master's degree from an accredited school or university in social or behavioral science or a related health specialty area.
Current State of IL Licensure as a Licensed Social Worker or a Licensed Clinical Social Worker.
ORThree (3) years (36 months) of progressively more responsible full-time experience in social work in a medical/clinical or social service agency setting.
