We are a Community Support (CS) Provider for various managed health plans for the California Advancing and Innovating Medi-Cal (CalAIM) program which allows Medi-Cal to pay for residential care facilities for the elderly (RCFEs): either large 100+ bed assisted living homes or small residential 4-6 bed homes and assigns participants to 5 different care levels. RNs with specific qualifications (see below) are required to administer the CS Assessment tool (we will provide training). CS participants are required to be on Medi-Cal and either be in a SNF for 60 days and be on LTSS or at home at risk of premature institutionalization.
The job will entail meeting in person with the CS applicant at either at his/her home, a RCFE, the hospital, or in a skilled nursing home and administering the CS Assessment tool. Total time needed for client care level assessment and filling out the needed paperwork required by the state is estimated to take 2 hours. A reassessment (once the client has been placed in a RCFE) is also required every 6 months. The job is extremely flexible and can work around your schedule. Many assessments can be done on weekends. The number of CS applicants requiring assessment will increase as both program mature through marketing outreach and referrals received from the state.
Pay: $60/hour for two hours (e.g.$130). Does not include travel time but we will also reimburse $15 for gas expenses daily.

The Registered Nurse (RN) will administer the Assessment Tool as developed for for CS Program and provide Care Assessment/Plan of Care Development services. We will provide complete training on the use of the CS Assessment Tool, the CS ISP, and training on the Care Coordination Procedures Manual, as provided by the DHCS.
The RN must have and maintain a current, unsuspended, unrevoked license to practice as an RN in the State of California. Work experience requirements include:
- A minimum of 1,000 hours of experience in an acute care setting providing nursing care to patients with similar care needs.
- A minimum of 2,000 hours experience in a home setting provider nursing care to patients with similar needs.
- Letter of references from at least 2 local skilled nursing homes
- Five years experience in conducting assessments, developing care plans, arranging for and monitoring service delivery, maintaining progress notes and case records, conducting quality assurance reviews, and collecting data as required.
Also required: own transportation and own computer/laptop.
