MAJOR DUTIES AND RESPONSIBILITIES
To provide direct nursing care to the residents, including treatments, under the medical direction of the resident’s attending physician. To supervise the day-to-day nursing activities performed by nursing assistants in accordance with current federal, state, and local standards, guide lines and regulations that govern our facility, and as may be required by the Director of Nursing Services or Nurse Supervisor to ensure that the highest degree of quality care is maintained at all times
Qualifications:
Must have a current, unencumbered, active license to practice as an LPN in New York State.
Must possess the ability to make independent decisions when circumstances warrant such action.
Must be knowledgeable of nursing and medical practices and procedures, as well as laws, regulations and guide lines that pertain to long term care.
Must possess leadership and supervisory ability and the willingness to work harmoniously with and supervise other personnel.
Duties and Responsibilities
Ensure that all nursing personnel comply with written policies and procedures established by the facility.
Meet with your assigned nursing staff, as well as support personnel, in planning the shift services, programs, and activities.
Participate in facility surveys made by authorized government agencies as may be requested.
Admissions, transfers and discharges residents as required.
Completes accident/incident reports as necessary.
Maintain the daily report.
Perform administrative duties such as completing medical forms, reports, evaluations, studies, charting, etc, as necessary.
Receive telephone orders from physicians and record as per facility policy.
Administer medications as ordered by the physician as per facility policy and procedure.
Ensure that controlled drug records are accurate for your shift. Count controlled drugs with oncoming shift.
Participate in orientation of new employees to your shift, the policies and procedures applicable to job position and duties.
Attend and participate in in-service education programs.
Monitor nursing personnel to ensure safety standards, and infection control policies and procedures are followed.
Maintain the confidentiality of all resident care information.
Know and comply with and ensure that all personnel respect resident’s rights.
Physical and Sensory Requirements:
Able to communicate with residents and personnel.
Able to remain calm under stress.
Able to read and write English.
Implementation of Basic Nursing Care
Assesses basic physical, emotional and social needs of residents.
Collects data for admissions assessment.
Develops and updates resident’s comprehensive care plans.
Assists residents with activities of daily living and encourages appropriate self care.
Safely perform therapeutic nursing techniques and skills as per clinical assessment.
Provides nursing care in conformance with established standards and policies and in keepingwith medical regimen.
Maintain accurate and complete records of observation of residents, nursing care provided and the resident’s response to care.
Teaches residents and significant others good health habits and documents accordingly.
Administer medications safely.
a. Transcribes doctor’s orders accurately on MAR.
b. Administers medication accurately.
c. Charts medications accurately.
Makes significant resident observations and communicates these to the health care team.
Maintains resident confidentiality.
Supports the resident in their expression of quality of life, resident’s rights and resident dignity.
Charge Nurse Responsibilities
Recognizes and responds to changes in resident’s conditions in a timely manner.
Communicates with doctor regarding changes in resident’s condition and accepts telephonemedical orders.
Maintains standards for compliance with NYS DOH LTC survey and participates in survey process.
Participates in the interdisciplinary team resident assessment meetings.
Completes incident reports and contributes to investigations as necessary.
Assigns personnel and directs day-to-day unit functions.
Participates in the documentation of monthly summaries and other documentation specific to long term care.
Reports unit and resident conditions and seeks guidance from the nurse manager and supervisor
Personnel Functions
Assists in orientation of newly assigned personnel as directed.
Interpret department policies and procedures to staff and visitors.
Creates and maintains an atmosphere of warmth, personal interest and positive emphasis, as well as a calm environment throughout the unit and shift.
