We are looking for full-time, part-time and PRN Nurse Practitioners and offer a flexible working schedule that we will tailor to meet your availability and desires. We are currently serving patients in the North Georgia counties of Gordon, Bartow, Whitfield and Murray counties.
Overview:
The Nurse Practitioner provides consultation in hospice or palliative care, symptom management and supportive care to meet patient needs in collaboration with Hospice or Palliative Care Medical Director, attending physician, and other members of the interdisciplinary team. Further, you will
- Provides medical support and coordination to palliative and hospice care patients and family members
- Provides medical expertise on pain and symptom management, medication review and the evaluation of terminal prognosis to patients, family members and staff
- Orders appropriate therapies and tests when needed
- Ensures that care is being provided based on established hospice and palliative care guidelines and standards
- Performs patient visits in private residences, long-term care facilities, assisted living facilities, hospitals, or other locations
- Consults with palliative care physician or designees as needed, informs primary care physician of services provided and collaborates with other physicians as needed
- Performs face to face encounters with hospice patients to identify/monitor existing or potential health problems and appropriateness of hospice recertification
- Attends interdisciplinary team meetings and reacts with other members of interdisciplinary team as needed
- Prepares and maintains accurate patient records, charts and documents to support sound medical practice and reimbursement for services provided
- Complies with applicable laws and regulations with respect to collaborative agreements
- Assists in the development of clinical practice guidelines/standards in support of quality hospice/palliative care
Qualifications:
- Advanced Practice Nurse license in Georgia.
- Registered Nurse licensed in Georgia
- Palliative Care certification (NBCHPN) or hospice certification (ACHPN) preferred
- Current CPR certification
- Previous experience in hospice or hospice-based palliative care program
- Must have reliable automobile, valid driver's license, and the minimum state required liability auto insurance.
