This position primarily works Monday-Friday 8am to 4:30pm, approximately 40 hours per week. This position also participates in a leadership on-call rotation, approximately every 6th week and every 6th holiday. IMPORTANT NOTE: Please be aware that our orientation process (typically up to 1 month) may require working days or shifts outside of what will be your permanent schedule.
What does an Admissions/Discharge Nurse do?
Under the direction of the Director of Nursing and in collaboration with other members of the rehabilitation team, the Admissions and Discharge Nurse has the following responsibilities:
- Collaborates with other rehabilitation team members, such as from Nursing ,Therapy, Case Management, and Social Work, to review potential admissions
- Collaborates with providers on admission orders and plan of care
- Arranges pre-admission needs such as room placement and equipment
- Responsible for the admissions process including report, order entry, admission assessment and care planning
- Ensures proper supplies and other specialty items are ordered to meet person served needs
- Responsible for admission paperwork and review of medications and care plans
- Assists with discharge process including discharge instructions and family education
- Collaborates with other members of the nursing leadership team to provide support and coverage for other leaders as needed

Qualified applicants must have:
- Current licensure in good standing to practice as a Registered Nurse in the State of Iowa
- At least 2 years of nursing experience
- Ability to maintain CPR certification
