The YWCA Greater Harrisburg is looking for individuals who want to make a difference in the lives of veterans. We are seeking an Outreach/ Employment and Supportive Services Case Manager to provide outreach, referral, coordination, case management and employment services to veterans impacted by opioid addiction to include those that are in housing, at-risk of losing housing or homeless to include: living in shelters, in transitional housing, in supported housing and in the community.
The Case Manager will be responsible for ensuring that veterans are assessed for housing, employment and supportive service needs; engaged and connected to Department of Veterans Affairs and community service providers, and as needed, receive case management services focused on meeting their recovery, self-sufficiency, wellness and housing stability goals.
Responsibilities include:
- Conducting outreach to service providers, agencies, organizations, and locations that provide access to veteran-specific and community-based, recovery, benefits, healthcare, employment, housing and other supportive services.
- Conducting outreach to local shelters, soup kitchens, medical outreach facilities, social service organizations, emergency shelters for families, and other locations where homeless veterans congregate, shelter or seek services
- Provide employment readiness services that support the veterans' educational, vocational, and career development goals. This includes utilization of best practices including access to Supported Employment, Customized Employment, job training and vocational programs
- Provide life skills to include: budgeting and financial goal setting as it relates to obtaining and maintaining long-term stability
Qualifications include:
- BA or BS degree in social work, human service or related fields of study including but not limited to Mental Health or Addictions, Social Work, Psychology and Vocational Development; or an AA or AS degree in equivalent field (s) with a minimum of 3 years' experience
- Ability to network within community and work with diverse population
- Excellent communication and interpersonal skills
- Proficiency in Microsoft Word and Excel
- Certified Peer Support and/or experience providing direct services
Job Type: Full-time
Pay: $18.31 per hour
