Clinical Pharmacist (Spanish Speaker) - Job Description
As the Clinical Pharmacist, you will be responsible for engaging patients and providing medication-related clinical services including but not limited to delivering chronic care management, remote patient monitoring, and enhanced Medication Therapy Management (MTM)services to our spanish-speaking patients via outbound and inbound communications that optimize therapeutic outcomes for patients.
Essential Functions and Responsibilities
- Make outbound calls to enroll patients.
- Provide disease and medication management services to patients via telephone or other telehealth technologies (such as videoconference).
- Perform comprehensive medication reviews, Care Management, to collect patient-specific information and assess medication therapies to identify medication-related issues and disease care gaps.
- Monitors drug therapy regimens for contraindications, drug-drug interactions, drug-food interactions, allergies, and appropriateness of drug and dose.
- Suggests appropriate, cost-effective therapeutic medication alternatives to medical staff.
- Communicate and work well with internal team members, community partners, and stakeholders to plan, facilitate, and execute clinical activities.
- Maintain proper documentation of clinical or care management activities in a proprietary software application.
- Provide education and training on the appropriate use of medications and monitoring devices and the importance of medication adherence and understanding treatment goals.
- Provide pharmacogenomic drug information, education, and training to physicians, nurses, patients, and other clinical staff.
- Provide clinical expertise to an interdisciplinary team for pharmacy and medication-related client activities.
- Remain current and knowledgeable about new initiatives, activities, opportunities, tools, and techniques through research, training, and continuous education.
- Providing clinical oversight into engagements so that delivery dates are met and ensure value is being delivered to the client as committed.
- Presenting progress and status updates to leadership
Required Education and Skills
- SPANISH SPEAKER
- Graduate from an American Council of Pharmaceutical Education accredited College of Pharmacy is required.
- Licensure or eligibility for licensure
- Board certification through the BPS (BCGP, BCPS, BCACP) or willing to become certified within 12 months of hire.
- Makes independent decisions within the scope of professional duties.
- Knowledgeable and current in all phases of pharmacy practice.
- Excellent interpersonal skills.
- Ability to manage multiple competing priorities.
- Proficient with Google Office Suite and Windows.
- Outstanding oral and written communication skills with the ability to influence others both internally and externally.

Job Types: Full-time, Part-time
Pay: $71,706.20 - $140,355.85 per year
Expected hours: 20 – 40 per week
