Responsibilities:
- Provides psycho-social assessments of patients to identify emotional, social, and environmental strengths and/or problems related to their diagnosis, illness, treatment, and/or life situation
- Provides patients with knowledge regarding the patient's diagnosis, illness and/or treatment
- Familiarity with hospital, medical center and/or health system resources to access and provide for patient care needs
- Participate in case review meetings
- Documents assessments plans, interactions, and interventions according to CNHF system guidelines and standards.
- Remain current with social work and health care developments
- Plan, develop, and facilitate educational activities for patients including but not limited to presentations, workshops, goal-setting and management, addressing techniques for health improvement, trouble areas, etc.
- Stays in compliance with all HIPAA regulations

Pay: $30.00 - $40.00 per hour
