Clinical Case Managers help keep patients in care and stable through the Clinical Case Manager's ability to effectively motivate, communicate and build relationships with patients while also consistently utilizing their above average skill of being highly organized and a proven ability to prioritize many tasks and competing priorities. Targeted Clinical Case Managers do the same, and also are responsible for enrollment of new patients in a unique medical setting, with additional responsibilities.

As a Targeted Clinical Case Manager, you are essential to the mission of Live Love Life, Inc. which is to reduce the transmission of HIV, STD's and Hepatitis C, by utilizing proven testing, motivation and treatment strategies and other interventions for those at risk. Your work makes a measurable impact on some growing community problems like STD infections, homelessness, IVDU, substance abuse, the opiate addiction epidemic, and rising new viral infections for HIV, hepatitis C and STD's.

We are looking for either an experienced Case Manager in our field or someone new to the field who has a strong desire to make a difference and who also has the proven skills to manage multiple priorities.
A Targeted Clinical Case Manager must have a commitment to get results as well as be highly motivated to work with our specific patient types. To be considered, the candidate must have proven organizational skills, be emotionally mature and possess a strong set of professional business acumen skills. A proven track record in delivering results in past work settings is required.
The best candidate is comfortable with and knowledgeable about substance abuse recovery, MAT, and if in recovery themselves has at least 2 hears sober and clea. The candidate must be impeccably punctual, works independently and maintains high result while working within a challenging specific and narrow set of rules. The successful candidate will be accountable for the full patient care cycle.

Targeted Clinical Case Manager duties include assessing, implementing, monitoring, intervening, leading and coaching clients to improved health through our program.
A certified or experienced Case Manager is a plus, but not required. A passion to help others, focused determination, emotional maturity and a powerful - self-starter initiative are essential.

Responsibilities
- Coordinate and provide all aspects of care.
- Handle case assignments, continually review case progress, facilitating next steps and determine case closure when necessary
- Facilitate multiple care aspects (case coordination, information sharing, referral to outside resources, risk, prevention treatment and disease education, etc.)
- Help patients make informed decisions by acting as their advocate regarding their care and treatment options
- Develop effective working relations and cooperate with medical team throughout both: the entire case management and clinical pathways
- Take the extra effort and go the extra mile building a professional relationship, trust and inspire patient progress.
- Record case information, accurately, complete all necessary forms and produce statistical reports
- Promote quality and cost-effective interventions and outcomes
- Assess and address motivational issues
- Routinely communicates and write direct and clear reports
- Contributes individually and enjoys a successful team over individual praise or glory
- Adhere to professional standards as outlined by protocols, rules and regulations
Major Functions/Accountabilities:
- Assistance accessing health insurance
- HIV testing and counseling, Hepatitis C testing and counseling, education and referral to preventive care such as with PrEP, or linkage to medical treatment for infection
- Assistance accessing medical care, including medications ie PrEP, Hepatitis C, substance use MAT, detox, rehab and recovery
- Client intake and assessment is primarily in the office, however as needed, inpatient hospital visits, outreach to specific partners, home visits and other strategies are necessary for enrollment and retention in care.
- Assistance with accessing support services: housing assistance, transportation, etc
- Referral for medical evaluation and treatment
- Education and counseling about HIV and hep C transmission, disease management, risk reduction and harm reduction education
- Medical case management of HIV medication therapy to include client education concerning risks and side effects, monitoring client adherence, and tolerance of medications
- Adherence assessment and interventions to include counseling, education, and referral as appropriate, including working with in-network pharmacy
- Educate on safer sex practices, clean needles to prevent transmission of sexually transmitted diseases and Hep C
- Assistance with other agency programs as needed including nights and weekend
- Other duties as assigned
Skills
- Proven working experience in case management, or a related job
- Project Management experience, education and/or skills.
- Excellent knowledge of case management principles, healthcare management and business savvy
- Confident with working with clients with a substance abuse history, and/ or HIV risk
- Ability to positively work effectively with others
- Effective communication skills
- Excellent organizational and time management skills
- Problem solving skills and ability to multi-task
Minimum Qualifications:
- Knowledge of the service needs of individuals affected by HIV/STI/AIDS, PrEP or Hep C who may or may not have a history of substance abuse
- Passion and commitment to help those at risk and in need of services
- Ability to thrive in a fast paced environment
- If candidate is in recovery, a minimum of 2 years clean and sober
- Ability to adhere to a zero tolerance drug use policy, and successfully pass random drug screening.
Preferred Qualifications:
- Comfortable working in different and varied work environments, such as a clinical setting, college campuses, sober living facilities, hospitals and homeless center settings to name a few.
- At least three (3) years of experience in working with organizations that serve individuals affected by hepatitis, HIV or STI
- Knowledge of PrEP, HIV care, Hep C care, opiate and other addiction recovery for substance abuse or alcohol
- A master’s/bachelor’s degree in non human services field and two (2) years case management experience
- A master’s/bachelor’s degree in a human services field
- Driver's license
