Speech Language Pathologist- Outpatient (PRN)

As the SLP, you will provide the best possible therapy services to patients consistent with established standards of practice and company and department procedures.
Responsibilities:
- Assure all treatment is delivered in accordance with an established plan of care.
- Provide clinical support and instruct patients, families, and caregivers.
- Monitor patient response to treatment intervention.
- Complete required forms and documentation in accordance with company policy and state/federal regulations
Requirements:
- Current licensure as Speech Language Pathologist in practicing state(s)
- Current ASHA Certificate of Clinical Competence
- Minimum of one (1) year as a treating therapist. Previous outpatient experience in the AL/IL settings preferred.
- Obtain NPI & PTAN number. Assistance provided in obtaining if not current upon hire.
- Enrollment in CAQH (Council for Affordable Quality Healthcare, Inc.) Assistance provided in enrolling if not current upon hire.
- Ability to exercise initiative and independent judgment.
- Current CPR Certification (BLS- Red Cross or American Heart Association)
- Understands regulatory compliance with state and federal regulations.
- Adhere to HIPAA and maintain client confidentiality.
