RN's are responsible for ensuring appropriate care for patients. RN is responsible for the quality of Joynus Care’s service. Thus, RN is focused on achieving the highest level of quality by becoming a great leader to staff she/he coaches.
- Train, direct and develop nurse aides, PCAs and other field staff
- Conduct semi-annual in-service training and skill test
- Visit patients during the time aide is present in the patient’s home.
- Review clinical records before seeing the patient
- Visit patients’ homes and assess client condition in adherence to Joynus Care policy and procedure and Georgia State rules and regulation
- Educate aides with a demonstration during assessments and supervisory visit
- Assess client medication to identify a possible ineffective drug or adverse reactions, significant side effects, drug allergies, and contraindicated medications.
- Educate aides regarding patient’s diet and medication including frequency and dosage
- Educate patients and their families on how to follow the care plan
- Provide clinical insight to Care coordinators and discuss with coordinators regarding care plan implementation
- Develop appropriate care plans and ensure aide is able to implement.
- Implement care plans by training, directing and coaching aides
- Assess environment safety to maintain patient safety
- Retain positive relationships with patients and their family members.
- Build a positive brand and image of Joynus Care
- Demonstrate professional conduct, service excellence and commitment to the values and mission of Joynus Care
- Retain positive working relationship with all staff of Joynus Care
- Communicate with patients
- Communicate with patient’s responsible parties
- Establish realistic measurable, observable goals consistent with the patient’s diagnosis
- Ensure that all records are in compliance with Joynus Care policies and procedures
- Comply with applicable legal requirements, standards, policies, and procedures including HIPAA.
RN/LPN is also responsible for Skilled Nursing Care
- Diet Education
- Meal Planning
- Medication Review
- Medication Management
- Infusion Therapy (I.V. medication administration)
- Wound Care Teaching and Management
- Medication Assistance and Teaching
- Medication Administration and Teaching
- Colostomy Care and Teaching
- Blood Pressure Teaching and Management
- Diabetes Teaching and Management
- Indwelling Foley Catheter Care and Management
- Assistance with finding Community Resources
- Any physician ordered services
Qualifications:
- Current GA RN license in good standing
- Minimum two years recent, verifiable clinical experience (home care and trach/vent/tube a plus)
- Strong assessment and organizational skills
- Solid communication and computer skills
- Valid CPR and First Aid certification
