Job Summary: This position is responsible for providing services to clients participating in the Career Track. This includes evaluating clients’ abilities, interests and skills related to employment, assessing clients’ need for vocational rehabilitation, basic work skills training and job training, working with clients to develop Individual Employment Plans, and referring clients to services
Essential Functions:
· Provide short term case management for clients, including crisis intervention and scheduled meetings.
· Initiate appropriate referrals if needed, if referral has not already been made.
· Encourage and monitor client attendance at suggested appointments and classes.
· Maintain accurate and timely documentation of client demographic and service data and all client contacts including meetings, telephone calls, collateral contacts, and internal referrals.
· Enter client data in HMIS, and Client Track in compliance with program and Funding Grant requirements.
· Become familiar with Housing First Model.
· Ensure that suspected child, adult and senior abuse and neglect are reported following established agency policy for non-emergency and emergency reporting procedures, and that the appropriate regulatory agencies are notified such as Child and Adult Protective Services.
· Maintain up-to-date client information.
· Perform other reasonably related business duties as assigned by immediate supervisor or other management as required.

Minimum Education and Experience: AA, or 2 years working as a case manage or like position with under-served populations.
Preferred Education and Experience: BA/BS in Human Services or related field that allows staff to work with adult and children, and experience working with under-served population
