Duties:
- Conduct comprehensive assessments of client's needs and develop individualized care plans
- Coordinate and monitor the delivery of services to ensure quality and timely care
- Collaborate with healthcare professionals, social workers, and other stakeholders to advocate for clients' needs
- Provide counseling and support to clients and their families to address mental health or addiction issues
- Assist clients in accessing appropriate resources and community support programs
- Monitor client progress and adjust care plans as needed
- Maintain accurate and up-to-date documentation of client interactions and progress
Experience:
- Minimum of 2 years of experience in case management or a related field
- Strong knowledge of medical terminology and patient care practices
- Experience in utilization management and coordinating healthcare services
- Familiarity with mental health counseling and addiction counseling techniques
- Understanding of behavioral health principles and interventions
- Knowledge of physiology, particularly in relation to pediatrics

- Excellent communication and interpersonal skills to effectively interact with clients, families, and healthcare professionals
- Strong organizational skills to manage multiple cases simultaneously
- Ability to work independently as well as part of a multidisciplinary team
- Proficiency in using electronic medical records systems for documentation purposes
