**Overview:**
We are seeking an experienced and dedicated Registered Nurse (RN) Service Consultant to join our team. In this role, you will play a crucial role in ensuring the delivery of high-quality patient care and medication management services. The ideal candidate will have strong clinical expertise, exceptional organizational skills, and a commitment to promoting patient safety and well-being.
**Key Responsibilities:**
1. **On-Call Coverage:** Provide on-call coverage as needed, responding to patient and staff inquiries and emergencies. Compensation of 0.25 hours for every call attended.
2. **Medication Administration Record (MAR) Reviews:** Conduct regular reviews of medication administration records to ensure accuracy, completeness, and compliance with prescribed regimens.
3. **Med Pass Observation Test Out:** Administer and evaluate medication pass observation tests to assess staff competency and adherence to medication administration protocols.
4. **Training on Medical Equipment:** Provide training to staff on the proper use and maintenance of medical equipment, including but not limited to infusion pumps, oxygen therapy devices, and nebulizers.
5. **Site Visits:** Conduct site visits to healthcare facilities and patient homes to assess medication management practices, provide support and guidance to staff, and address any issues or concerns.
6. **Medication Destruction:** Ensure proper disposal of expired or unused medications according to established protocols and regulatory requirements.
7. **Medication Cabinet Organization:** Maintain medication cabinets in accordance with client preferences, separating and organizing medications by client and distinguishing between topical and oral medications.
8. **Narcotic Count:** Perform regular audits and counts of narcotic medications to ensure accurate documentation and prevent diversion or misuse.
9. **Medication Supplies Ordering:** Coordinate the ordering and replenishment of medication supplies to ensure adequate stock levels and minimize disruptions in patient care.
10. **Appointment Checklist Review:** Review appointment checklists to verify that all necessary medications and supplies are available and prepared for patient visits.
11. **Appointment Summary Visits:** Participate in patient visits to review medication regimens, address medication-related concerns, and provide education on medication management and adherence.
12. **Medication Orders:** Process medication orders, including prescription refills and new medication requests, in collaboration with healthcare providers and pharmacies.
13. **Therap Medication Monitoring:** Monitor Therap medication levels to adjust dosages and treatment plans as needed.
14. **Geritom Interface:** Utilize Geritom or similar software for medication management, ensuring accurate documentation and communication of medication-related information.
15. **Facilitate Medication Classes:** Conduct medication classes for patients and caregivers to educate them about proper medication administration, potential side effects, and the importance of adherence to treatment regimens.
16. **Training and Competency Building:** Train and assess staff members to ensure competency in medication administration and management practices.
**Qualifications:**
- Current Registered Nurse (RN) license in good standing.
- Bachelor's degree in Nursing (BSN) preferred.
- Minimum of 4-6 years of experience preferred in a skilled, assisted, or community group facility.
- Strong knowledge of medication safety principles, pharmacology, and regulatory requirements.
- Excellent organizational, communication, and interpersonal skills.
- Ability to work independently and collaboratively in a fast-paced healthcare environment.
- Proficiency in electronic health record (EHR) systems and medication management software.
- Commitment to providing patient-centered care and promoting positive patient outcomes.
