In collaboration with the clinical team, the Residential LPN will work with homeless adults who are diagnosed with serious behavioral health disorders, and/or those who may have a co-occurring substance abuse disorder. The Bridge has a strong commitment to providing evidence-based and best practices that emphasize health and wellness throughout its continuum of outpatient, community support, and residential services.
This position is Monday to Friday, 9 am to 5 pm. One of the two openings offers a flexible schedule within set parameters at a shelter/ Safe Haven. It involves working at multiple sites in a shared office and coordinating with other medical staff.
The Licensed Practical Nurse (LPN) is an integral member of a multi-disciplinary team. This position requires a highly organized, detailed, and assertive individual who is flexible and willing to work in a high-demand environment. The position requires direct care work assisting clients with minor medical issues and advice, referrals, and performing training for both staff and clients.
Essential Position Functions:
- Assist consumers in accessing medical care.
- Monitor medication, ensure documentation quality, and build rapport with pharmacies.
- Monitor vaccinations and health initiatives.
- Collaborate with clinical staff, managers, supervisors, and program personnel.
- Perform administrative tasks, including correspondence, reports, and record-keeping.
- Support adults diagnosed with serious mental illness in The Bridge supportive housing.
- Document consumer interactions in electronic medical record systems like AWARDS and CARES.
- Make and follow up on referrals, act as a liaison with healthcare providers, and provide consumer education.
- Assist in crisis management and report incidents or disruptive behavior.
- Attend meetings, conferences, and training sessions.
- Maintain professional boundaries while interacting with clients and adapt to flexible duties, locations, and schedules.
- Any other duties may be assigned.
Required Knowledge, Skills, and Abilities:
- License and current registration to practice as a practical nurse in NYS.
- Three years of experience working with individuals with mental illness and/or co-occurring substance use disorders.
- Experience in a culturally diverse environment and experience understanding various evidence-based models and theories pertaining to psychoeducation, motivational interviewing, mental illness, and recovery.
- Highly organized and self-motivated. Proficient in computers.
- Bilingual Spanish a plus but not required.
