Job Title: Director of Patient Care Services - Home Health
Overview:
We are looking for an experienced DPCS to join our team! Our top priority is to ensure our patients receive the best care in the Sacramento region. As the Director of Patient Care Services for Home Health, you will play a crucial role in overseeing and coordinating all patient care activities within the home health agency. This leadership position requires a combination of clinical expertise, managerial skills, and a commitment to ensuring the highest standards of patient care.
Responsibilities:
Leadership and Management:
- Provide strategic direction and leadership to the home health team, including nurses, therapists, aides, and administrative staff.
- Provide training and ongoing support to clinical staff on agency procedures, regulatory requirements, and best practices.
- Foster a collaborative and supportive work environment that promotes professional growth and development among staff members.
Clinical Oversight:
- Ensure the delivery of high-quality patient care services in accordance with established care plans and physician orders.
- Conduct regular assessments of patient needs and collaborate with healthcare professionals to develop individualized care plans.
- Monitor patient outcomes and satisfaction levels, implementing improvements as necessary to enhance the quality of care provided.
Regulatory Compliance:
- Stay abreast of changes in regulatory requirements and ensure compliance with all applicable federal, state, and local regulations governing home health care services.
- Oversee the preparation for and participation in regulatory surveys and audits, taking corrective action as needed to address any deficiencies identified.
Interdisciplinary Collaboration:
- Work closely with other healthcare providers, including physicians, hospitals, and community organizations, to facilitate coordinated care and continuity for patients transitioning between care settings.
- Participate in interdisciplinary team meetings and case conferences to ensure comprehensive care planning and coordination.
Qualifications:
- Bachelor's degree in nursing or related field required.
- Current licensure as a Registered Nurse (RN) in CA.
- Minimum of 4 years of progressive leadership experience in home health setting.
- Strong clinical knowledge and understanding of home health regulations and standards of practice.
- Excellent communication, interpersonal, and problem-solving skills.
- Demonstrated ability to lead and motivate a diverse team of healthcare professionals.
- Proficiency in computer applications and electronic health record systems.

Hours:
- Monday-Friday 8:00 a.m. to 5:00 p.m.
- Remote on-call, 1-2 weekends a month
Working Conditions:
The Director of Patient Care Services for Home Health typically works in an office environment within the home health agency. Travel to patient homes and other healthcare facilities may be required occasionally. This position may involve some evening and weekend hours to accommodate patient needs and staff schedules.
