Job Summary:
The assisted living facility in Bensonhurst is seeking a dedicated and compassionate individual for the position of Director of Patient Services. Director of Patient Services is responsible for the daily planning, organizing, and evaluating Patient service programs and support functions. The ideal candidate is a licensed and currently registered professional nurse with at least two years supervisory experience. This position reports to administrator.
Responsibilities:
· Hire, orient, and supervise all professional staff reporting to position.
· Coordinate and integrate care provided by facility staff, including contractual providers.
· Demonstrate competency with the State Survey process and ensure that the agency as a whole and all the agency staff are prepared for on-site surveys and other external audits.
· Conduct Clinical Quality Improvement (CQI) functions and report to the Quality Improvement Committee (QIC).
· Review infection control reports for all clients. Provide all follow up related to infection control reports (both client and employee), process these reports in appropriate database and ensure clinical managers and supervisors are reviewing for their teams.
· Collaborate on the development and implementation of policies and procedures that guide and support the provision of care.
· Assign work to staff, ensure patients records are current, complete, and care provided in accordance with established treatment plan.
· Ensure ongoing and appropriate clinical supervision including documented competency and annual performance evaluation for all staff.
· Identify staff education needs, prepare annual in-service plan and secure appropriate training resources.
· Implement system to monitor patient satisfaction, including complains and incidents; review and follow-up on all QI reports; maintain administrative logs.
· Act as liaison between nursing and billing staff to ensure timely submission of claims to third party payors.
· Demonstrate availability of self or designee at all times in person or via telecommunications.
· Ensure that clients’ Plans of Care are executed as written.
· Ensure a reassessment of a client's needs is performed by the appropriate healthcare professional when there is a significant health status change in the patient's condition.
Qualifications:
· Currently licensed and registered to practice as an RN by the New York State Education Department
· BSN Degree
· Knowledge and experience of UAS
· Knowledge and experience of Assisted living NYS regulations.
· Minimum of 2 years previous experience in Assisted living facility or Home Care
· Minimum of 2 years experience as a Supervising Community Health Nurse
· Demonstrate outstanding leadership skills and interpersonal skills.

Pay: $110,000.00 - $120,000.00 per year
