About the job
Waypoint's Health, Well Being and Independence Program (formerly Home Care) in the Manchester, NH area is currently hiring a part-time Registered Nurse. Our team of three RNs, led by a Nurse Manager, works autonomously to manage a caseload of clients during typical business hours - no weekends, evenings, or holiday hours required! Join our team of compassionate staff who assist seniors in achieving their goal of remaining safely in their homes. This is an amazing opportunity to be a part of a mission driven program. You will work with the client to develop care plans that may include:
- Conducting a nursing assessment on the client and their environment
- Helping the client with their medications
- Assessing and educating the client about their diagnoses and management
- Identifying in-home services needed
- Providing Case Management with involved providers
- Offering community resources

Qualifications:

You must be motivated and passionate about our mission and purpose

A Licensed Registered Nurse. Home Care experience with seniors is preferred.

Working knowledge of computers and willing to learn Electronic Visit Verification software.

Ability to complete thorough & timely documentation.

Strong interpersonal communication skills & an active listener.

Familiarity with community agencies and programs serving the greater Manchester area.

The chosen candidate will be asked to have a criminal background, motor vehicle record check, and have a valid driver’s license & auto insurance that must meet company standards.
