Primary function is to administer skilled nursing care, under the supervision of a registered nurse for patients in their place of residence, coordinate care with the interdisciplinary team and patient/caregivers. This position will promote patients' potential during their home health episode and collaborates with physician, clinical team, and patient in preparation for discharge.
General Responsibilities
· Provides direct patient care as defined in the Texas Nurse Practice Act.
· Implements plan of care initiated by the registered nurse/physician.
· Provides accurate and timely documentation consistent with the plan of care.
· Assesses and provides patient and family/caregiver education and information pertinent to diagnosis and plan of care.
· Participates in coordination of home health services, appropriately reporting the identified needs for others to the registered nurse and/or Clinical Manager.
· Uses equipment and supplies effectively and efficiently.
· Participates in personal and professional growth and development.
· Participation in Quality Assurance/Performance Improvement Program and Home Health Agency sponsored training.
· Performs other duties as assigned by the registered nurse.
· Provide care utilizing infection control measures that protect both staff and patient (OSHA).
· Records treatments administered in Careficient.
· Records clinical documentation according to accepted regulatory, corporate, and professional guidelines.
· Participates in weekly case conferences.
· All documentation will be completed within 24 hours of visits, 7 days per week.
Skills
· Graduate of an accredited practical nurse or vocational nursing program.
· Three years nursing experience
· Currently licensed as a licensed practical nurse or licensed vocational nurse in the state of Texas.
· One year prior professional nursing experience in Home Health.
· Complies with accepted professional standards and practice.
· Demonstrates good verbal, written communication, and organization skills.
· Must be a licensed driver with an automobile that is insured in accordance with state and organization requirements and is in good working order.
· Possesses and maintains current CPR certification.
Educational Background
· Associate or bachelor's degree in healthcare administration, nursing, or a related field. A master's degree is preferred.
· Previous experience in a healthcare or administrative role is often desired.
· A strong understanding of healthcare terminology, regulations, and practices is essential.
Requirements
· Must have working knowledge with various EMR systems, such as Careficient.
· Excellent interpersonal skills and ability to communicate effectively with the Altruist team, patients, and referral sources.
· Ability to commute to the Dallas office and be onsite. This is not a hybrid role.
Required Physical Activities
Approximately 95% of time is spent working in the field.
Must have reliable transportation.
Clear diction and acute hearing are necessary for effective communication with co-workers, customers, and outside agencies, by telephone and in person.
Ability to effectively use the computer screen and interpret printed materials, memos, and other appropriate paperwork.
The ability to transport files and office supplies.
The ability to stoop and kneel is necessary for accessing files/stocking supplies.
Good manual dexterity is necessary for computer keyboard use and for retrieving and working with appropriate paperwork, equipment, and supplies.
