DUTIES OF THE POSITION:
Under general supervision of clinical management, you will be providing skilled nursing care to patients and directing the care of our LPN's and HHA's to provide quality care. Asses, plan implements, evaluates total patient care, and coordinates/family teaching. Assures compliance with nursing standards federal, state, and local government regulation, and company clinical policies. Provides goal directed care leading to specific outcomes. May be on call for after hours work.
Duties:
- Conduct comprehensive assessments of patients' health status and develop individualized care plans
- Administer medications, including injections, as prescribed by physicians
- Perform specimen collection and processing for laboratory testing
- Provide wound care, monitor vital signs, and assess patients' overall condition
- Collaborate with interdisciplinary healthcare team members to coordinate and manage patient care
- Educate patients and their families on disease management, medication administration, and self-care techniques
- Utilize medical terminology and documentation systems accurately and efficiently
- Provide basic life support in emergency situations
-Provides nursing care to assigned patients
-Assesses patient's physical and emotional status, level of competency, home environment, safety factors, and family support available to assist with care and needs of the patient
-Provides information on patient rights, reimbursement procedures, and patient/family rights
-Observes patient for changes in condition and communicates/coordinates care with attending physician or allowed practitioner and clinical management
-Responsible for planning, evaluation and treatment implementation of assigned caseload
Skills:
- Valid Registered Nurse (RN) license in the state of Pennsylvania
-At least one year of experience in home health preferred
-Ability to work independently and manage time effectively
-Strong interpersonal, organizational, and problem solving skills
-Strong solid computer skills, prior experience with Home Care Homebase preferred
