Duties:
- Perform nursing assessments and provide direct patient care
- Perform triage over the phone to determine if patients need to be seen in office, be scheduled for a telemedicine visit, or can be treated at home
- Collaborate with healthcare team members to develop and implement care plans
- Educate patients and their families on health conditions, treatments, and self-care
- Maintain accurate and up-to-date medical records using MedConnect
-Respond to medication refill requests from the phones and through the EHR
Skills:
- Experience working with children and individuals with special needs
- Proficiency in documentation practices
- Familiarity with durable medical equipment (DME) and its proper use
- Strong communication skills to effectively interact with patients, families, and healthcare team members
