Position Description: Supports Planner/Case Manager
The Supports Planner is accountable for providing high-quality Supports Planning Services to consumers, in accordance with MAC policies and procedures, Independent Living philosophy, and the requirements of both funding sources and regulators within the geographical areas of the lower shore Maryland Counties (Dorchester, Somerset, Wicomico and Worcester Counties).
This position reports to the Support Planning Supervisor.
This is a full-time position with benefits: 35 hours per week starting at $19.50/hr.
Essential Functions:
· Provide Supports Planning (SP) services to clients referred by the Maryland Department of Health (MDH).
· Assess the client for their need for services and supports.
· Support the client’s ability to choose service providers and self-direct the services they receive, to the extent applicable.
· Use the MDH LTSS database system to develop a Plan of Service (POS) in accordance with MDH regulations to support clients who are leaving nursing facilities or who need supports to maintain their independence in the community.
· Learn and follow all applicable policies and procedures for Support Planning services and POS development/management, as established by MDH, as well as comply with timelines and mandates specified by MDH.
· Make phone contact with clients on a monthly basis. Have face-to-face contact with consumers on a quarterly basis, and document in LTSS that contact was made.
· Display sensitivity to the client’s ethnic, religious, racial, and disability-related culture, and to the client’s general living environment.
· Coordinate and facilitate meetings with the client, family members or concerned others, and service providers.
· Meet regularly with the Supports Planner supervisor to discuss consumers’ needs and receive direction on work activities
· Attend orientation and ongoing in-service trainings
Qualifications:
· Bachelor’s Degree in the Human Services field: Social Work, Psychology, Sociology, Nursing, Counseling, or a related field, with coursework pertaining to older adults or to adults with disabilities.
· Excellent interpersonal, problem-solving, and conflict resolution skills.
· Strong project management skills with the ability to manage and prioritize multiple tasks and work autonomously under pressure of deadlines.
· Strong verbal and written communication skills.
· Ability to take initiative and consistently exercise good judgment.
· Knowledge of the Maryland Social Services system
Physical Requirements:
· Prolonged periods of sitting at a desk and working on a computer
Other Requirements:
· Criminal background check required prior to employment
· Valid driver’s license
· Regional travel is a requirement of this position (lower 4 counties)
