Position Summary
The Speech Therapist is a licensed professional responsible for providing therapeutic intervention in the rehabilitation and restoration of residents through the use of assessment and treatment procedures/modalities.
Qualifications:
- Must possess a Master’s degree in Speech-Language Pathology from an accredited university program
- Must hold current Certificate of Clinical Competence (C.C.C.)
- Possesses a current Pennsylvania Statelicense
- A.S.H.A. membership strongly preferred and encouraged
- C.P.R. certification strongly preferred and encouraged
- One to two years experience preferred in a long-term care/sub-acute care as S.L.P.
- Clinical expertise preferred in geriatric speech-language disorders and dysphagia
Essential Duties and Responsibilities
1. Participates in the screening and/or evaluation of all incoming resident admissions within 72 hours of admission and regular screening of all residents to ensure that they are maintaining the highest level of communicative, swallowing, hearing, and cognitive ability.
2. Completes assessments and diagnostic procedures in order to develop treatment plans.
3. Provides speech language pathology services, resident-specific treatments, and therapeutic interventions in accordance with the treatment plan of the attending physician.
4. Plans and conducts discharges of residents, including resident and caregiver education, and making recommendations to the interdisciplinary team regarding discharge and appropriate placement of the resident in the health system continuum of care or to the community.
5. Provides consultation and regular in-servicing to staff and family caregivers, and to improve community awareness and understanding of Speech-Language-Pathology and rehabilitation services
6. Develops specialized Speech-Language-Pathology programs of the facility in cooperation with other departments to continually strive to improve overall quality of resident’s care
7. Provides appropriate referral services to ensure continued quality services (i.e. audiological services, Modified Barium Swallow Studies, ENT consultation, etc.)
8. Serves as a member of the interdisciplinary team and facilitates co-treatment with OT and PT staff
9. Documents results of assessment, treatment, follow-up, discharge, and billing notes in an accurate, timely manner in accordance with Standards of Practice, state and federal regulations, and policies/procedures
10. Provides clinical supervision and instruction of Speech-Language Pathologist in their clinical fellowship year, students, interns, rehab techs, and volunteers
11. Commits to high ethical standards and complies with the Code of Conduct, state and federal regulations, accreditation standards and Concordia policies and procedures. Maintains confidentiality of resident and employee records and information. Has a duty to report compliance concerns, significant legal risk questions, fraud, abuse, suspected or actual misconduct to a supervisor/manager or to the Corporate Compliance Officer.

Pay: $30.00 - $45.00 per hour
