At Home Harmony transforms the care for aging seniors and enables them to thrive in their personal home or in a senior living community. The organization provides a set of recurring home care services - primary care medicine, social work, mental health, personal care, remote patient monitoring, and pharmacy with prescription drug delivery - in an integrated model that drives improved healthcare outcomes and patient/caregiver satisfaction. We connect the dots between providers, medications, care management, and loved ones who want to support their elderly or disabled family members.

This position is not remote and requires on-site presence and entails up to 25% travel and one Saturday monthly:
Responsibilities:
- Evaluate physician medication orders for appropriateness of drug, dosage, potential interactions, route of administration and stability.
- Makes recommendations for therapy changes to the physician and/or facility staff.
- Documents all communication with the healthcare team and patients/caregivers
- Maintains knowledge of pharmacy laws and federal and state regulations
- Works cooperatively as a member of team
- Maintains knowledge of pharmaceutical care practices, specifically as related to geriatric based medicine
- Works directly with Pharmacy Director to maintain compliance and expand pharmacy services
- Verify and dispense medications to clients and provide counseling on medications received
Qualifications and Requirements:
- Bachelor’s degree from an accredited School of Pharmacy or Pharmacy Doctorate
- Current license required (multiple state licenses preferred)
- One-year experience in acute, long-term care or home health preferred
- Must be able to work weekends or outside of normal business hours when needed
- Good customer service skills
- Understand state and federal regulations for retail, in home and long term care pharmacy operations
- Familiar with Athena, Rx30 and Outcomes MTM preferred
- Knowledge of chronic care management (CCM) – preferred
- Geriatric/Senior Adult expertise - preferred
Pay: From $58.00 per hour
Expected hours: 1 – 20 per week
