Community Options, Inc. is a national non-profit agency providing services to individuals with disabilities in 12 states.
We are actively seeking Full-Time and Part-Time Day Program Staff in West Valley City, UT. This role provides individualized services designed to assist participants in managing and overcoming the difficulties confronted by an individual with a disability living in the community.
Starting Pay Rate is $19.00 per hour
We are offering a $1,000 sign-on bonus for full-time staff and a $500 sign-on for part-time staff (16+hrs a week) eligible for this bonus after 90 days of satisfactory employment. We are also offering a $700 employee referral bonus for a full-time employee and a $350 employee referral bonus for a part-time employee (employee must meet 90-day satisfactory work/attendance requirement).
Responsibilities
- Support and encourage individuals to increase their ability to be as independent as possible.
- Must be able to use own operating vehicle
- Working one-on-one with one client that are apart of our staff.
- Meet with waiver participants on a regular basis- either in the individual’s residence, the community, or an office setting.
- Collaborate with the Case Manager to develop and facilitate programs which meet the social, emotional, physical and personal needs of the individual.
- Provide ongoing support for integration efforts.
- Document individual’s progress toward acquiring community integration.
- Assist in building a support network between the individual and the community.
- Ensure medical consultation forms are completed, by the physician, for each medical appointment attended.
- Administer medication(s) as needed and ensures required documentation is completed.
- Coordinates all leisure and volunteer opportunities.
Requirements:
- High School Diploma/GED required.
- Valid Driver’s License.
