QUALIFICATIONS
1. Six (6) months of Certified Home Health verifiable aide care in the home, hospital or related employment.
2. Successful completion of a Certified Home Health Aide training program as recognized by the State of California for certification.
3. Current Certified Home Health Aide Certificate issued by the State of California.
4. Current CPR Certification.
5. Is fluent in English.
6. Must be able to communicate effectively, both verbally and writing.
7. Able to walk, bend, stoop, and lift objects weighing up to 25lbs.
8. Must be licensed driver with an automobile that is insured in accordance with state and/or organizational requirements and is in good working.
9. Must have dexterity, coordination, visual, and auditory acuity to perform all job responsibilities.
10. Current satisfactory report on pre-employment physical examination including Mantoux Tb Test or chest X-ray as required by agency policy and procedures.
POSITION SUMMARY
Performs various patient care activities and related services necessary in caring for the personal needs and comfort of patients within the scope of practice, physician orders and as assigned in writing and supervised by the Registered Nurse or Therapist appropriate.
ESSENTIAL DUTIES AND RESPONSIBLILITIES
1. Helps patient with bath, care of mouth, skin, hair and/or other personal hygiene activities.
2. Helps patient in and out of bed.
3. Assist patient with ambulation use of wheelchair and/or walker as directed.
4. Helps with prescribed exercises which patients and home health aide have been taught by appropriate health personnel.
5. May prepare meals and help patients with eating.
6. Performs household services which facilitate the patient’s self-care at home and are necessary to prevent or postpone institutionalization.
7. Helps patients to bathroom or in us of bedpan/urinal.
8. Reports changes in patient’s condition and/or needs to supervising nurse and appropriate therapist.
9. Works independently in home with only intermittent RN supervision.
10. Charts record of patient care on progress notes after each visit.
11. Completes appropriate records.
12. Provide safe and sanitary environment for the patients.
13. Participates in team conferences and educational activities.
14. Performs all activities incorporating and applying the principles of proper body mechanics.
15. Participates in orientation, staff education, team meetings and patient care.
16. Implements care based on the Plan of Care.

Qualifications:
- High School Diploma or equivalent preferred.
- Valid driver's license with acceptable driving record.
- Must be able to read, write, speak, and understand the English language.
