Position Overview: We are seeking a detail-oriented and customer-focused Eligibility Specialist to join our team. In this role, you will be responsible for evaluating and determining applicants' eligibility for various programs and services. Your primary duties will include reviewing applications, verifying documentation, and maintaining accurate records. About the Program: The MI Choice Waiver Program (The main program you will be working within), formerly known as HCBS/ED, offers Medicaid-covered services akin to those in nursing homes but allows eligible adults to remain in their own home or community setting. Established by Michigan Medicaid in 1992 and available statewide since 1998, it provides basic Medicaid services along with support coordination and various additional services, including adult day health care, counseling, nursing services, and more, catering to individuals' specific needs and promoting independence
Qualifications:
- Associates Degree in a related field preferred
- High School Diploma/GED and 1 years of experience (required)
- Experience in working with older adults and/or persons with disabilities preferred.
- Previous Experience working in administrative support or human services roles.
- Proficiency in standard office applications and computer literacy required (Microsoft 365 products).
- General understanding of long-term care, person-centered thinking, and community resources preferred.
- Ability to handle and prioritize multiple tasks is essential.
Benefits:
- Full-time position (37.5 hours/week), no weekends or on-call duties.
- 50/50 hybrid schedule after training period
- Flexible scheduling between the hours of 6am and 7pm Monday- Friday
- $200 continuing education/ per year

Key Responsibilities:
- Telephones and/or travels throughout Berrien, Cass, and Van Buren Counties to visit participants in their homes and other institutional or community-based settings to assist in preparation, collection, and submission of eligibility and entitlement applications and redeterminations.
- Completes additional screening for wait list applicants to determine program eligibility.
- Corresponds and coordinates with agency staff, participants, their representatives, businesses, community service partners and providers, and others to access and collect information and documentation needed to complete entitlement applications and redeterminations and access Third-Party Liability benefits.
- Performs and/or assists with completion and submission of entitlement applications and redeterminations and all subsequent follow-up responses to local office inquiries for additional information or documentation.
- Notifies local DHHS office of enrollments and disenrollments and maintains accurate program status in appropriate software system(s).
- Tracks timelines and completes timely redeterminations to ensure seamless continuation of benefits.
- Reviews discrepancies in program enrollment and works with the local DHHS office and MDHHS staff to resolve issues as needed.
- Provides support to applicants working toward financial eligibility. Connects applicants with community services as appropriate.
- Organizes and maintains Medicaid and Third-Party Liability resources, forms and applications.
- Provides technical Medicaid information and assistance to agency staff, participants, and partners.
- Maintains current, accurate, and HIPAA compliant participant files and program records.
- Assists as needed with other projects and assignments that optimize program delivery and implementation.
- Other duties as assigned.
