Job Summary:
We are seeking a highly skilled and experienced Director of Case Management to join our team. The Director of Case Management will be responsible for overseeing and managing the case management department, ensuring the delivery of high-quality patient care and effective utilization of resources. This role requires strong leadership skills, clinical expertise, and the ability to collaborate with interdisciplinary teams.
Duties:
- Develop and implement case management policies, procedures, and protocols
- Supervise and provide guidance to case managers, nurses, and other staff members
- Conduct regular performance evaluations and provide feedback to team members
- Collaborate with healthcare providers to develop individualized care plans for patients
- Monitor and evaluate patient progress, ensuring that care plans are followed effectively
- Collect and analyze data related to patient outcomes and utilization of resources
- Provide guidance on behavioral health interventions and therapy techniques
- Ensure compliance with regulatory standards and managed care requirements
- Conduct documentation reviews to ensure accuracy and completeness
- Collaborate with other departments to improve patient care processes
Requirements:
- Bachelor's degree in nursing or related field (Master's degree preferred)
- Current RN license (or equivalent) in good standing
- Minimum of 5 years of experience in case management or related field
- Strong knowledge of managed care principles and practices
- Excellent leadership skills with the ability to motivate and inspire a team
- Proficient in data collection, analysis, and reporting
- Strong organizational skills with attention to detail
- Excellent communication and interpersonal skills
