We are looking for a special, talented, and experienced Client Care Coordinator to join our Scottsdale office team.

The Client Care Coordinator is a central and key team member in the agency. This role is primarily responsible for working directly with our clients, family members and our amazing caregiver field staff, ensuring care needs are coordinated and scheduled properly. They work closely with the other Client Care Coordinators and Recruiting as part of a team, as well as having individual responsibilities. They report to our Manager of Operations.
This position requires exceptional communication skills and flawless follow-up. Strong prioritization and problem solving skills are needed, with the ability to work in a fast-paced environment. Every day will bring something new and different – and we celebrate our wins together!

Valid driver’s license and background check required.
KNOWLEDGE, SKILLS, & ABILITIES

- Excellent communication skills, with a great telephone presence
- Ability to work independently and as part of a team, and perform under tight deadlines
- Superior customer service and problem-solving skills
- Creative thinking to channel ideas and energy into a pleasant office atmosphere
- Attention to details and strong work ethic
- Ability to handle sensitive and confidential information
- Solid office computer skills required with light data entry. Wellsky/ClearCare experience desired but not required
- Minimum of 1-3 years of relevant work experience. Case Management, Home Care, Hospice, or Home Health background a plus.
- Sense of humor is a must!
RESPONSIBILITIES
- Handle all client and caregiver inquiries efficiently and professionally
- Create and maintain schedules for both clients and caregivers
- Coordinates care needs and resolve gaps in client’s care, scheduling and related issues
- Understand the nuances of assigning appropriate caregivers to clients to ensure the highest level of care is provided
- Works closely with other Care Coordinators, Client Services and Field Supervisors to ensure the care needs of the clients are understood
- Helps ensure key office policies and compliance procedures are followed by field staff
- Maintains required documentation and filing for clientele and care staff
- Participate in rotating call coverage with other agency staff
- Bring some sunshine to the office and leave the “attitude” at home.
PREFERRED EDUCATION:
- High School or equivalent required
- Associate or Bachelor's degree preferred
