Alaska Bleeding Disorder Clinics is the only federally recognized Hemophilia Treatment Center in Alaska. The Hemophilia Treatment Center in Alaska has long had a reputation for its outstanding relationship with the local bleeding disorder community and with the local patient advocacy group, Alaska Hemophilia Association AHA. Many of the core board members of AHA were heavily involved in the creation of this clinic, and the expectation is that most patients in the Alaskan bleeding disorder community will choose to receive their bleeding disorder care at our new clinic, being well aware of the great benefits of being cared for in a Hemophilia Treatment Center.
**Job Title:** Nurse Coordinator
**Job Summary:**
The Hemophilia Treatment Center (HTC) Nurse Coordinator provides expertise in care coordination for children and adolescents with bleeding and clotting disorders. This role requires advanced clinical knowledge and skill to partner with the multidisciplinary care team, which includes, but is not limited to, hematologists, pulmonologists, nutritionists, social workers, ophthalmologists, nephrologists, psychologists, and community health workers. The HTC Nurse Coordinator, in collaboration with the patient care team, will evaluate patient care delivery and provide clinical coordination with specialists across the entire health system.
**Responsibilities:**
-Participate in each comprehensive clinic as a full assessment team member, including the post-clinic discussion, identifying and talking with the patient and family regarding their special needs, and providing follow-up with patients.
-Provide specialized assistance to patients/families that arise during and between comprehensive clinic visits.
--Regularly participate in team conferences, training and program development
-Demonstrate understanding of the treatment issues in this population through maintenance of education and skills. RN should have training that includes: hemophilia physical assessment skills; infusion skills; patient/family education training.
-Nursing assessment of general health, as well as bleeding disorder-related problems
-Provide culturally sensitive, language-appropriate bleeding disorder outreach and patient/family education, including education to lay population and professional groups
-Coordination with home infusion and pharmacy
-Development and coordination of individualized patient treatment plan based on provider recommendations, sent to primary care provider, patient/family and updated at least annually.
-Development of individualized emergency treatment plan based on provider recommendations, sent to primary care provider and patient/family and updated at least annually.
-Development of travel letter based on treatment plan, provided to patient/family and updated at least annually.
-Serves as liaison between HTC and the community, including primary care providers. Identifies appropriate providers and facilitates and coordinates patient services.
-Coordinate lab testing and provide results
-Coordinate care needs related to surgery, orthopedics, physical therapy, genetics, infectious disease, hepatology, obstetrics, gynecology, pharmacy, and dental health providers.
-Provide education and training in home and self-infusion, including central venous access devices and indwelling catheters.
-Triage urgent patient calls both during and after business hours as needed. Promote emergency department avoidance through same-day visits, if available and appropriate. If advising patient be seen at emergency department, call report to ED prior to patient arrival.
-Triage and Follow-up with patients regarding acute bleeding events and treatment management
-Document nursing interactions via EMR. Collaborate and participate in regular staff meetings.
-Coordinate with the Non-profit association and attend specific events as determined by the Clinic's needs. Events include, but are not limited to, Kids’ Camp, Women’s Retreat, Annual Education Day, and fundraising events.
