AMG Hospice is currently seeking a Full Time Registered Nurse (RN) to work as part of the core interdisciplinary group. The RN will play an important role in providing holistic and patient centered care to the patients and families that we serve. Strong clinical and communication skills combined with a strong sense of compassion are required to ensure the highest quality of life for patients as we assist with their end of life journey.
The responsibilities of the RN include but are not limited to:
- Conducting a comprehensive assessment of patients' physical, emotional, and psychosocial needs, while collaborating with the interdisciplinary group to develop personalized care plans
- Coordinating and managing patient care and ensuring that interventions are consistent with patients' wishes, goals, and the hospice philosophy
- Providing skilled nursing care, pain and symptom management, and emotional support to patients and their families in accordance with protocols
- Collaborating closely with the physicians, social workers, chaplains, and other team members to address the needs of patients and their families
-Communicating effectively with patients and families to provide education about the hospice process and supporting informed decision-making
- Monitoring the patients' condition, updating care plans as needed, and ensuring timely documentation in electronic health records
-Participating in regular interdisciplinary group meetings while contributing insights and expertise to enhance the delivery of patient care
Requirements:
- Active REGISTERED NURSE (RN) license in good standing with the AZ state board of nursing
- At least 1 year of experience working in Hospice/ Palliative care
- CPR certified
- Must have a valid driver's license and be willing to travel to patient's home locations in Phoenix, Tempe, Scottsdale, Chandler, Mesa and Gilbert

Pay: $95,000.00 - $100,000.00 per year
