Responsibilities:
What You Will Do:
The Practice Manager of Home-Based Primary Care is responsible for the day-to-day operations of the Primary Care Service line, aiming to improve outcomes, drive growth, and enhance profitability. This role involves strategic planning, administrative coordination, program development, and implementation across retirement, nursing, skilled nursing, and other healthcare facilities. The Manager will also oversee home-based primary care programs and demonstration projects, serving as a liaison with physicians, and coordinating quality and compliance activities.
Key Responsibilities:
Strategic Planning and Service Line Oversight:
- Assist in the development and monitoring of strategic plans for home-based primary care.
- Collaborate with the Director to implement business plans, evaluate competition, and ensure alignment with the company's strategic goals.
- Work with marketing staff to promote home-based primary care.
- Develop performance reports and dashboards with relevant indicators.
- Lead community outreach plans to fulfill service line goals and the company's mission.
Clinical and Administrative Operations:
- Monitor operating budgets for programs and services.
- Facilitate improvement activities to enhance efficiency, productivity, revenue, and cost containment.
- Maintain ongoing communication with leadership within each branch and crescent of care.
Quality Assurance and Compliance:
- Oversee quality/performance improvement and compliance activities.
- Develop and maintain data management systems for clinical, process, satisfaction, and cost outcomes.
- Ensure adherence to CMS, accreditation, and regulatory standards.
- Facilitate successful site visits and accreditations.
Physician Engagement and Enrollment:
- Develop and maintain collaborative working relationships with physicians.
- Assist in the implementation of strategic plans for home-based primary care.
- Collaborate on physician engagement and monitor provider contracts.
Team Management and Development:
- Manage performance, conduct performance appraisals, and ensure staff complete required training.
- Monitor staffing, turnover, and vacancies, taking necessary actions to maintain adequate staffing levels.
- Foster a culture of continuous improvement and professional development.
Service Excellence:
- Identify and address customer needs, ensuring high standards of service.
- Collect and monitor patient/family satisfaction surveys and implement recovery systems for service failures.
Qualifications:
Formal Education Required:
- Master’s degree in business administration, healthcare administration, nursing, social work, or a related field.
Experience and Training Required:
- Three years of relevant experience managing a medical office/practice, including two years of supervisory experience.
Skills, Competencies, and Qualifications:
- Ability to work collaboratively with an interdisciplinary team.
- Independent judgment and critical thinking skills.
- Effective communication skills, both verbal and written.
- Competency in Microsoft Office and electronic medical records.
- Ability to process new information quickly and efficiently.
- Flexibility to adapt to changing needs.
Physical Demands:
- Sedentary: Exerting up to 20 pounds of force occasionally and negligible force frequently.
Performance Metrics:
- Development and maintenance of a relational database for outcome analysis.
- Training team members on the data management system.
- Implement and maintain quality metrics to track performance and results.
- Engage and enroll physicians, maintaining collaborative relationships.
- Develop strategic plans and business plans for service line growth.
Working Conditions:
- Travel within the region to oversee operations and meet with teams or stakeholders.
- Flexibility to respond to urgent issues outside of standard working hours.
