Position Summary:
The Hospice Nurse will be responsible as a member of the interdisciplinary and clinical team for the implementation, delivery and coordination of hospice services. Hospice Nurse will provide direct patient care including personal care/activities of daily living according to the plan of care for hospice patients as assigned and complete documentation accurately and timely. The Hospice Nurse will be responsible for at least 1/3 of on-call after hours, weekends, and holidays – may be ½ of on-call. The Hospice Nurse will meet the performance objectives for the position. This position MAY HAVE supervisory responsibilities of LPNs.
Qualifications:
Minimum education and experience required:
· ADN nursing
Preferred education and experience:
· Two (2) to Three (3) years’ experience as a hospice or long term care nurse preferred
· Three (3) to Five (5) or more years of long term care experience as a CNA/CMA/LPN
· Proficiency with electronic medical record software
Minimum licensure or registration required:
- Valid drivers’ license
- Valid Kansas nursing license
- BLS/CPR
Preferred licensure, registration, certification:
· BSN nursing
Minimum knowledge, skills and abilities required:
· Ability to develop constructive and cooperative working relationships with others and maintain them over time.
· Ability to solve practical problems and deal with a variety of variables in situations where some guidance and standardization exists.
Preferred knowledge, skills and abilities:
· Ability to manage all aspects of end-of-life care with expertise in pain and symptom management and hospice nursing.
Essential Duties and Responsibilities:
· Complete required training by assigned date
· Complete continuing education and renew licensing/certification
· Coordinate planning and delivery of interdisciplinary interventions in concert with team members
· Provide leadership for interdisciplinary team meetings; the development, review and revision of the patient/family plan of care; facilitating problem-solving sessions; and delivery of team support
· Ensure delivery of safe and efficient quality hospice services by utilizing the nursing process within available resources
· Assist with patient personal care
· Document accurately and timely – point of care charting
· Respond to patient & family needs for 1/3 of on-call after hours, weekends, and holidays
· Supervise LPNs as assigned
· Engage in regular and periodic reviews with supervisor. Accepts direction with a positive attitude.
· Communicate effectively both orally and in writing.
· Communicate with office – office will plan schedule and enter into EMR
· Participates in committees, task forces and agency events as required
· Attends staff meetings, IDT Team and other meetings as assigned
· Complete other duties as assigned
