Care Management Support Rep
- This is a Call Center Position.
- Will go onsite first day to pick up equipment.
- Monday to Friday 8am to 5pm. Training will be virtual 8am to 5pm for 3 weeks
This is a HYBRID role-Local to Phoenix, AZ candidates only.
Job Description:
-Coordinates care with providers as a part of comprehensive and proactive discharge planning.
-Identifies and escalates quality of care issues through established channels.
-Utilizes critical thinking and judgment to collaborate and inform the case management/concurrent review process, in order to facilitate appropriate healthcare outcomes for members by providing care coordination, support and education for members through the use of care management/concurrent review tools and resources.
-Will assist with discharge plans, and outreach to members, assist with Home Health/DME issues.
Required:
* Minimum of 1 year in a customer service or call center environment
* Experience with Case management and Discharge Planning.
*Computer experience is required with proficiency in Microsoft Outlook, Word and Excel.
* Basic alpha number data entry skills with attention to accuracy and quality is essential.
* Minimum Typing Speed: 45 wpm AND Minimum Keyboarding: 6500 alphanumeric (Assessments will be given)
Education:
High school diploma or GED required

Pay: $17.50 - $18.60 per hour
