SRO Housing Corporation is building a strong case management team for our social services division that offers mental health case management to individuals living in our affordable housing property on Skid Row in downtown Los Angeles. Job Duties Include:
Provide intensive case management services to previously homeless clients/residents who present with such needs as health care issues, mental health matters, substance abuse issues, financial matters, social support concerns, HIV/AIDS, etc. Maintain assigned caseload and assess client/resident needs. Assist clients/residents in developing an appropriate Individual Action Plan aimed toward meeting their individual needs. Maintain and update all client/resident records and progress note all interactions. Conduct follow-up activities necessary to assist clients/residents in the implementation and modification of their Individual Action Plans. Assist with linking clients/residents to SRO Housing programs and services as well as other community agencies and programs. Prepare reports for management.
Requirements
Education: A Bachelor's Degree/BSW or higher in mental health or human services field and preferably three or more years social service experience in a nonprofit environment. Prefer specific training/experience working with homeless or formerly homeless persons who have co-occurring conditions such as a mental illness, HIV/AIDS, substance addiction, physical health conditions, etc.
Required Skills:
Experience/knowledge related to working in the mental health field. Proficient with written and oral communication skills. Must have computer skills, specifically Microsoft Word, Excel,Outlook, Clarity, HMIS. Comfort working with a diverse population, including clients with severe and persistent mental illnesses. Comfort working independently and with a multidisciplinary team in a fast paced environment. Ability to maintain productivity and documentation standards. Knowledge of Los Angeles County Department of Mental Health Records paperwork. Compassion for clients, ability to manage time efficiently, prioritize tasks, demonstrate sound judgment, give attention to detail, be flexible, self-driven and motivated. Strong networking and interpersonal skills.

Pay: $22.00 - $24.00 per hour
Expected hours: No less than 40 per week
