Job Summary: LVN Case Manager is responsible in handling case management of home health patients and the assigned staff team to ensure effective and quality delivery of home health care services in accordance with the State and Federal Regulations and the Agency's Policies and Procedures. He/she coordinates plan of care, patient care and physician's orders with assigned team members of Registered Nurses, Licensed Vocational Nurses, Medical Social Workers, Certified Home Health Aides and Rehab Services.
Knowledge, Skill, and Experience:
- LVN with current license to practice in the State of California
- Minimum of 1 year of home health case management experience preferred
- Knowledge of home health regulatory and reimbursement requirements preferred; sensitivity toward Medicare population required
- Computer skills required for both clinical and clerical programs; With computer and Microsoft Office Skills; Must be able to type a minimum of 45 words per minute and have a working knowledge on medical terminology
- Excellent verbal and written communication skills
- Ability to work independently on assigned tasks as well as accept direction on given assignments
- Ability to supervise and direct members of home health care team
- With organizational and time management skills
