Position Title: Admissions RN/Hospital Liaison

As an Admissions RN/Hospital Liaison, you will coordinate and plan admissions, provide information to health care providers, and provide skilled nursing assessment care to the patient as prescribed by the attending physician or designee and in compliance with the policies and procedures of Centrica Care Navigators.
Here’s what you will be doing:

Receive referrals from hospital case manager, RN, social worker, or physician and notify medical records and billing.

Assess patient’s condition and past medical history, conduct initiation visits and obtain appropriate documentation to support hospice eligibility.

Work with admissions coordinators and SOC RNs to provide information on planned or possible discharges daily.

Serves as a clinical consultant to hospital staff regarding hospice care.

Answer calls, enter referrals, update reports, assign hospital liaisons and maintain visit calendar.
Work schedule:

3 12-hour days (8 am-8 pm)

Weekend and holiday rotation
Requirements:

Graduate of an accredited school of professional (registered) nursing.

Maintain license as a Registered Nurse from the State of Michigan.

Minimum of one-year general nursing experience with current Hospice, medical, surgical, or critical care experience.

Current BLS certification.

Experience with issues concerning the terminally ill, and death/dying, and pain and symptom management.

Maintain a valid Michigan driver’s license and the availability of a reliable, licensed and insured vehicle for use on the job, vehicle must be insured in accordance with the State of Michigan.

Negative TB skin test and/or chest X-ray and other tests as required by Centrica Care Navigators.

Up to date on all routine immunizations required by LARA and agency policy.
