JOB DESCRIPTION
POSITION: INDEPENDENT LIVING SPECIALIST
STATUS: FULL TIME
RESPONSIBLE TO: EXECUTIVE DIRECTOR
DUTIES: Essential responsibilities of the job are to provide effective and efficient IL services to consumers; including provision of all appropriate OFA CIL services, to include, but not be limited to:
1. Establish and maintain consumer files as outlined in case management guidelines.
2. Documentation of time spent on the planning/provision of IL services and resulting impacts/outcomes of such services for each assigned consumer file.
3. IL services will be provided to consumers in their homes/communities when it facilitates the consumer's independent living goal attainment.
4. Coordinate other agencies/service providers who could assist consumers in goal progress/attainment.
5. Function within the scope of the C.R.C. Code of Ethics.
6. Advise the Executive Director of circumstances that might warrant attention/services beyond the scope of OFA CIL purposes(s) and/or operational policies and procedures.
7. Attending meetings, conferences, and workshops at request of the direct supervisor or the Executive Director.
8. Perform other duties as assigned by the direct Site Manger or the Executive Director.
9. Maintain complete confidentiality of any and all information that concerns consumers, potential consumers and OFA CIL staff. FAILURE TO DO SO WILL CONSTITUTE JUST CAUSE FOR IMMEDIATE TERMINATION OF EMPLOYMENT.
QUALIFICATIONS: An Associate's degree in Rehabilitation, Social/Human Services, or a related field, or significant experience in related activities. Substantial consideration will be given to persons who have a sound knowledge of Independent Living philosophy, and experience in the delivery of services to persons with disabilities in Independent Living Centers or similar work settings.
